Medical Forms & Information

First-Year Athletes - [Rookie Medical Form]
This includes all athletes new to the University of Regina athletics program (includes transfer students). All first year athletes are required to have a complete medical examination by a physician and completion of the Rookie Medical Form by the physician. The athlete is responsible for any costs incurred for the completion of the form. The form must be completed in full, signed, and submitted to the Health Care therapist or team trainer prior to participation in any practices or training camps. Alternatively, the completed form can be delivered in person or faxed directly to the Allied Health Centre (306-585-4417).

Returning Athletes - [University of Regina Annual Health Reassessment Form]
This includes athletes who have competed for the University of Regina over the past competitive season and have a Rookie medical form on file at the Allied Health Centre. All returning athletes are required to submit a completed University of Regina Annual Health Reassessment Form. Upon review of this form by the head therapist and/or the physician director of athletics, athletes may be required to be reviewed by a physician to gain medical clearance for participation. The University of Regina Rams Football Club require an annual medical for new and returning athletes.

Participation in University athletics may result in physical injury and/or in the aggravation, deterioration and/or re-injury of any pre-existing medical condition(s) during and after one’s involvement with the University of Regina. It is important that athletes fully understand and assume the risks of participating in University athletics. The University of Regina and University of Regina Athletics will not pay for expenses that are normally covered under any other applicable insurance or government plan. Athletes participating with a University of Regina Athletics team must have a valid Provincial Health Care/Hospitalization Number AND have medical coverage through a private / 3rd party insurance policy. Athletes who do not have the above health care insurances will be denied participation. University of Regina full time students are required to carry the URSU Student Health Care Plan, or provide proof of alternate third party plan to be eligible to opt out of the University plan. To qualify for the URSU Health Plan, you must be considered a full time student. Any online classes do not qualify toward full time status. Health Care Services for Athletes

Allied Health Centre Services – Centre for Kinesiology, Health & Sport
Physiotherapy / athletic therapy services are available to all University of Regina intercollegiate athletes as well as the general public. Physiotherapy / athletic services for Cougar athletes will be subsidized by Cougar Athletics from August 15 – April 15. Services between April 15 and August 15 will be available on a fee for service basis. Athletes may be able to claim physiotherapy costs from their health care insurance plan. Please note individual health plans vary in the extent of physiotherapy coverage benefits.

The Physiotherapy Centre sees athletes by appointment only with the exception of Monday afternoon (12 – 2 pm) and Tuesday morning (9 – 11 am) drop in hours during the competitive season only. Appointment hours are posted in the Allied health Centre. To make an appointment, call 337-2641.

Medical Services are available in the Allied Health Centre. All physicians accept patients by appointment only. Last minute appointments are available on a daily basis and are arranged on a first- come first- served basis.

Other services in the Allied Health Centre such as chiropractic, massage, podiatry, and traditional Chinese Medicine can be accessed by athletes on a fee for service basis. Such services are arranged by appointment only.

Insurance Coverage
Basic Health Coverage: Athletes must have a valid Provincial health number from their home province or equivalent coverage. Athletes from outside of Canada who have not established their eligibility under the Hospitalization and Medical Care Acts should arrange through the University for Insurance Coverage to be effective throughout the waiting period (3 months). More information can be obtained at www.uregina.ca/gencal/ugcal/servicesForStudents/ugcal_124.shtml under 8.4.10.2 - "Health & Accident Insurance."

Student Health & Dental Plan: Cougar Athletics requires athletes to carry additional insurance beyond their provincial health care plan. The University of Regina Students’ Union (URSU) sponsors a Health and Dental plan. All undergraduate and graduate students who are URSU members and who are taking 9 or more on-campus Fall credit hours are automatically enrolled in the Plan. Athletes who already have equivalent coverage may opt-out in order to avoid being charged the mandatory fee as long as they give proof of alternative coverage. This may include BlueCross, Group Medical Services or another company provided by parents or spousal employers. The amount of coverage is dependent on the individual plan. Some plans are dependent on age and marital status and may change without notification. For complete details, go to http://www.ursu.ca and click on "Services" and "Student Care Health & Dental Plan." PLEASE BE AWARE THAT IT IS YOUR RESPONSIBILITY TO ENSURE YOU HAVE COVERAGE EITHER THROUGH THE URSU STUDENT HEALTH PLAN OR A PARENT/SPOUSAL PLAN.

Student Accident Benefit Plan: Under the Student Accident Benefit Plan, certain expenses that arise out of accidents and injuries sustained while participating in authorized University and related activities are paid by the University. The plan is intended as a supplement to the Saskatchewan Medical Care Insurance Act and not as an alternative to it. Copies of the regulations under which such payments may be made are available from Financial Services, the Faculty of Kinesiology and Health Studies and at www.uregina.ca/presoff/vpadmin/policymanual/students/902040.html.

Sport Medicine & Science Council of Saskatchewan (SMSCS) is a partner to the University of Regina and offers services that may not be available at the Allied Health Centre. The SMSCS services include nutrition counselling, sports psychology, sport biomechanics and. exercise physiology.
